Firefly Hot Yoga Barre – Minor Participant Waiver

Acknowledgment of Risks and Voluntary Participation (For Minor Participant)

I, the undersigned, am the parent or legal guardian of the child named in this registration and am completing this waiver solely on their behalf.

I understand that participation in yoga, barre, and fitness classes—including those offered in-studio, outdoors, virtually, or via recorded video—carries inherent risks. These activities may include stretching, strengthening, cardiovascular exercise, balance work, and the use of props such as blocks, straps, bands, hammocks, or other equipment.

I acknowledge that my child is voluntarily participating in programs, classes, workshops, or events offered by Firefly Hot Yoga Barre (“the Studio”) and that such participation may involve physical exertion that could result in injury or health complications.

I affirm that my child is in good physical condition and has been cleared by a healthcare provider to participate in such activities, or I have knowingly chosen not to seek such clearance and accept responsibility for that decision.

I understand that the Studio’s instructors are not medical professionals and cannot diagnose conditions or prescribe treatment. Any instruction provided is intended for general educational purposes and is not a substitute for medical advice or care.

I, on behalf of my child, myself, our heirs, executors, and assigns, voluntarily waive and release any and all claims against Firefly Hot Yoga Barre, its owners, staff, instructors, and contractors for injuries, illness, death, property damage, or any other loss that may arise from my child’s participation—regardless of cause, including but not limited to negligence.

I understand that entering the Studio or participating in in-person events may carry an increased risk of exposure to contagious illnesses, including COVID-19. I voluntarily accept this risk on behalf of my child and agree to hold Firefly Hot Yoga Barre and its agents harmless from any related claims.

I understand that photos or videos may be taken during classes or events for marketing or promotional purposes. I grant the Studio permission to use my child’s image, likeness, or voice in such materials without compensation.

If I do not wish for my child to be photographed or recorded, I will notify a staff member prior to the start of class or event.

Parent/Guardian Consent and Agreement

By checking the box and submitting this form, I confirm that I am the parent or legal guardian of the minor participant and that I have the legal authority to grant this consent. I have read, understood, and voluntarily agree to this waiver and release on behalf of my child. I acknowledge that this is a legally binding document, and I agree not to pursue any legal claims against Firefly Hot Yoga Barre related to my child’s participation.
